What You Need To
Know This Month

Teton County Public Health embarks on Community Health Needs Assessment with St. John's Medical Center using the Mobilizing for Action through Planning and Partnerships (MAPP) Framework.

Share your ideas about the health and quality of life in Teton County through a brief survey by Healthy Teton County. The survey is part of Healthy Teton County’s "Community Themes & Strengths assessment," which gathers residents’ views on the area’s most pressing health issues.

The 5-minute survey is available online at
Healthy Teton County. The survey can also be accessed via QR (quick response) codes on flyers posted in local establishments. Additionally, a paper version of the survey will be available at community events and meetings throughout the summer.

Healthy Teton County is a community health initiative led by Teton County Public Health and St.John’s Medical Center, with participation from over 25 local organizations.

Three other assessments will take place over the next two months as part of a broad community health needs assessment. Following completion of all four assessments, Healthy Teton County will publish a comprehensive report on its findings as well as a community action plan to address identified health needs.

Information on the MAPP Framework and how you can get involved can be found at healthytetoncounty.org
