Water System Questionnaire

Dear Water Lab Client:

Teton County Environmental Health is now utilizing a database which allows us to organize and store bacteriological water analysis information for both Public and Private water systems and retrieve sampling records more easily. To set up a water system in the database, we must input such information as would be gathered in a sanitary survey into the database. Additionally, the EPA has indicated that it would like us to start transmitting water lab results electronically, and this database will allow us to do so.

We currently do not have the necessary information about many of the Public Water Systems that we regularly monitor for bacteriological safety. We have developed the following 3 page form that would supply much of the needed information, and are asking that you, along with anyone else who is familiar with the water system, please fill in as much information as you can. If you have a sanitary survey for your system, please include a copy of that as well when you return the form.

We appreciate your help in our effort to update our records. You can either mail the completed forms to us or bring them to Teton County Environmental Health, which is located on the lower level of the Teton County Public Health building.
