Human Services Agencies

This data collection was designed with two purposes in mind; for use by potential clients or referral agencies to find appropriate services, and for agencies to use for collaborating on grant writing, projects, and information reporting.

The data matrix includes: agency contact information, a list of the types of services offered, information collected by each agency as well as the populations served by each of the participating Human Service providers in the Teton County “Systems of Care”.

The matrix was designed to be a quick reference as well as easily accommodate updates and additional participants. Data included in the matrix is updated yearly.
